The registration requirements are:

1. Provide one form of identification for yourself
2. Provide one document for each member of your household
   (Acceptable documents include (but are not limited to):
   Wisconsin's immunization registry record
   Report cards
   Birth or baptismal certificates
   Passports
   Medical, Insurance, or Library Cards
   Drivers' Licenses
   State/military/tribal IDs
   Social Security Cards
   AND
   a first class mail to a member of your household postmarked within the past 30 days
3. Your current address must be verified by providing one of the following:
   First class mail postmarked within the past 30 days
   Rent receipt
   Lease agreement
   Property Tax Bill
